The first part of this verse speaks about thoughts, and the second part speaks about words, but the two are closely tied together. Thoughts are words to God, and words are judged by the thoughts that produce them. So the thoughts and words of the wicked, which are wicked like they are, and which aim at harm or have some evil effect, are disgusting to the Lord. He is offended by them, and he will call them to account.
This is true because God knows the heart and everything that rises up in it. He does not just see what people say and do, he sees what they think as well. He also requires the deepest and highest place in the heart, meaning that he wants the whole inner life to belong to him.
The thoughts and words of the pure, being pure like them, are clean, honest, and sincere. These are pleasant thoughts and pleasant words, and they are pleasing to the holy God, who loves purity. This can refer both to their worship of God and to their conversation with other people.
In prayer and praise, the words of the mouth and the meditations of the heart are acceptable to God (Psalm 19:14; Psalm 69:13). Their speech with others is also pleasing when it aims to build people up. Both are sweet to God when they come from a pure, purified heart.
